They just offered to refund what I'd paid for the original online reservation and seemed to think that solved everything. Super Shuttle had no explanation for why this had happened. Execucar had received reservations from Super Shuttle and had just never scheduled the pick ups, so other travellers either missed or came close to missing their flights as I did. Apparently I was not the only person that this had happened to. Fortunately my flight was delayed, so I made it.
The driver got me to the airport in the nick of time. I was frantic and asked them to send someone right away to get me to the airport, which they did. There had been some lack of communication between Super Shuttle and the local company and apparently the local company had received my reservation but never scheduled my pick up. They said they had no record of my reservation and referred me to the local company that is contracted by Super Shuttle Salt Lake City called 'Execucar'. It was getting close to my flight's departure time and I finally called Super Shuttle. I stood waiting outside my hotel and the van never came! I paid by credit card and received a confirmation and a pick-up time. Pre-paid shuttle to SLC airport from my downtown hotel never came! I made the reservation online with Super Shuttle 24 hours in advance.
